Term & Condition for Institutional Clients

1. Fill completely and sign Institutional Account Opening Form and Institutional Account Settlement Form
2. Enclose Letter of Incorporation with all of the changes which is approved with Ministry of Law and Human Rights which show Board of Director, Board of Commissioners, and the last shareholders
3. Enclose copy of Tax Payer Identification Number ( NPWP ).Those without NPWP must submit do not have NPWP letter.
4. Enclose Sign Company List (TDP), Enclose Domicile Certificate (SKD)
5. Enclose Trade Business License or other Business License according to client business, Enclose Financial Report for the last 3 years
6. Enclose ID Card of Management Company, Enclose ID Card of owner or shareholders
7. Management Structure & Ownership Structure, Power of Attorney (if necessary)
